When the silicone implant is placed either above or below the pectoral muscle. The procedure takes about two hours and is performed under general anaesthesia. The implant is embedded via a small micro-fine incision in the lower breast fold. Subsequently, the skin is sutured with self-dissolving threads.
There are two different implant shapes, round and teardrop-shaped. The teardrop-shaped implant is very similar to the natural breast shape. However, the final result depends heavily on the anatomy of the body. The size and shape of the implant are individually adapted to the patient's body shape and wishes.

A reconstruction of the breast with breast implants can be performed directly, under the same anaesthesia after removal of the mammary gland. The implant is inserted either in front of or behind the pectoral muscle. However, sufficient skin and soft tissue mantle must still be present. The method is not suitable for patients with large, contracted scar plates. This is often the case after irradiation. In some cases, a preliminary operation for skin extraction is necessary to prepare for the implantation, for which an expander is inserted. This is continuously filled with a saline solution and thereby expands the skin coat. If the skin or soft tissue coat is large enough, the actual implant can be inserted. One Breast reconstruction with breast implant offers above all the advantage that the procedure is rather small. The disadvantage is the use of foreign material and the associated risk of hardening of the breast, especially after radiation (capsular contracture).
With the help of autologous tissue flaps of the back or abdomen, the breast can be rebuilt. If the flap is removed from the back, the back muscle (latissimus dorsi) is partially or completely lifted off with skin on one side. There is only a scar on the back. The loss of muscle is usually well compensated by other muscles. The so-called DIEP cloth is a further development of the TRAM flap. Here, the abdominal muscles are completely preserved. This reduces the risk of abdominal wall hernias.
The nipple and areola are built up as the last breast element. The procedure can be performed under local anaesthesia or, if necessary, with corrective surgery after breast volume has been restored.
